Great Southern Region - Albany

Home and Community Care funding for a neurological community nurse was approved in 2003 and the first nurse appointed to this position.  The NCWA is now looking to consolidate and build this service to meet the needs of patients and carers affected by neurological conditions.  This is a free service for people with neurological conditions.

This service includes:

• Home visitsAssessment of conditions
• Liaison with GPs, specialists and other health professionals
• Information and education about neurological conditions
• Respite service advice
• Support group information and education
