This is a new HACC funded service for people with neurological conditions which has just been established (in May 2006). This service includes:

  • Home visits
  • Assessment of conditions
  • Liaison with GPs, specialists and other health professionals
  • Information and education about neurological conditions
  • Respite service advice
  • Support group information and education
  • Clinics
