If you, or someone you know, has a neurological condition or symptom and would like to access our neurological nursing services, depending on your circumstances, you can do this via one of two ways:
- As a Department of Health funded Neurocare nursing services client (e.g. over 65 years or ineligible for NDIS), or
- As a National Disability Insurance Scheme (NDIS) client.
Both options are provided at no cost to you.
The Neurological Council of WA is a registered NDIS provider of community nursing care. We provide the same high-quality practice standards to all our clients.
We take referrals from:
- General practitioners
- Health professionals
- Community service workers
- Regional assessment agencies
- Families and friends of potential clients
- Primary carers
- You can also make a self referral.
Are you eligible for NDIS funding? You may choose the Neurological Council to provide your community neurological nursing services. If so, the level of service will be negotiated with you and clearly stated in your Service Agreement with us.
For more information or to chat to one of our neurological nurses about your individual circumstances, please phone 1800 645 771 or email firstname.lastname@example.org.
For more information about the NDIS, including ‘Am I eligible?’ and ‘How do I apply? please visit the NDIS website.
